Facility/Vehicle Use & Room Reservation Form
Members and groups having an established relationship with First United Methodist Church of Salisbury are permitted to use the grounds and facilities for gatherings and activities which are consistent with the mission of the Church. Those seeking to use facilities must complete a Facility Use & Room Reservation Request Form and obtain approval in advance of use. Generally, requests for the use of facilities during church business hours (Monday-Friday: 9am - 3pm) are approved by the Business Administrator in accordance with this policy. Requested use of the facilities outside normal business hours requires approval of the Board of Trustees.
Effective July 01, 2023: Outside groups using church facilities are expected to contribute toward the cost of utilities and cleaning in accordance with the following fee schedule:
-A101 (Stanback Room): $25/hour
-Chapel: $50/hour
-Fellowship Hall: $50/hour
-Kitchen: $25/hour
-Other Classroom Space: No Charge
(Additional donations to help offset costs would be accepted.)

Payment of Facility Rental Fees must be remitted prior to facility use, once approval is granted. (Online payments may be made at
www.fumcsalisbury.org/donate
or a check made payable to First United Methodist Church.)
*The Board of Trustees reserves the right to waive fees for certain applicants at its discretion; and/or approve certain groups to substitute a service project benefitting the church in lieu of rental fees.
*Please NOTE: Alcohol and tobacco products are strictly prohibited on FUMC property at all times.*

TERMS AND CONDITIONS FOR BUILDING USAGE: *I understand that I am responsible for the setup and clean-up of the facilities and will ensure they are returned to normal setup, following the approved event. Following the event, I will collect all garbage into bags and dispose of it in the dumpster. I will wipe off all tables with mild soap and water (or other appropriate cleaner). I will return all tables, chairs, easels, and other materials or equipment to its proper place after the event. I will remove and dispose of all leftover food and drink items. If I use the kitchen, I will ensure all dishes are washed, dried, and properly stored, and that the floors, counters, and equipment are all cleaned before vacating the premises. I will remove and return all decorations and materials to their rightful storage places (taking care not to damage the walls.) I will ensure the restrooms are left clean and orderly and that all trash is disposed in the dumpster. I will ensure that all building entry doors are secured before vacating. I understand that tobacco and alcohol are not permitted on FUMC premises. I will report any damages/stains to the Church Office promptly. The person/organization requesting the use of church facilities hereby absolves First United Methodist Church of Salisbury, its pastors, staff, leadership, members, or people of any liability for personal injury to any individual resulting from the use of the church facilities and agrees to be responsible for any property damage that may result during the use of the facilities.

VEHICLE USAGE TERMS AND CONDITIONS Keys may be checked out and returned in church office during office hours (Monday - Thursday, 09:00am - 3:00pm), or arrangements can be made for after-hours pick-up. Please ensure the bus is in clean and working condition, refill the gas, and notify the Church Office of any mechanical problems or issues with the vehicle. You are responsible for purchasing gasoline and paying any traffic violations, fees, or fines, incurred during your possession of the church vehicle. The Board of Trustees is responsible for the care, routine maintenance and repairs, and control of the church vehicles, and for the implementation and review of all policies and guidelines governing their use.

(The driver of the bus must be at least 25 years old, hold a valid North Carolina Driver's License, and a satisfactory driving record. For insurance purposes, please list the person who will be driving the bus and provide a copy of his/her driver's license. The only persons authorized to drive the church vehicles are those on the "Approved Drivers List." You may inquire of the church office to secure an approved driver from the list.)
